Interested in becoming a new patient?
If you are looking to schedule an appointment for an evaluation with Even Keel Health, this is your place.
NEXT STEPS
There are a few simple steps to schedule a new patient appointment.
Step 1:
Complete the New Patient Registration
Make sure you hit submit
Before you begin,
make sure you have about 10 minutes,
access to your insurance card,
and any prior documentation files such as school reports, IEP/504, or old testing and evaluations

Step 2:
Once the "New Patient Registration" paperwork above has been completed and submitted, the patient will be registered in our system (EHR).

As soon as you hit Submit, you will receive an email with instructions on how to register for the portal.
Step 3:
Register for the portal and hit "request access"
Use the same name (patient's) and email you used in the The New Patient Registration form above.
You will automatically be given access to the portal once you register.
You can not register for the portal unless you are already in the system as a patient.
Step 4:
Once you registered for the portal, you now have access to the patient portal.
Log into the portal and schedule a New Patient, in-person appointment (adult or child)

Step 5.
Once you have scheduled a new patient appointment, the final packet of information will be emailed to you to complete,
"Even Keel New Patient Questionnaire"
Patients must complete all required paperwork at least two days prior to their appointment to allow staff time to review the patients information and history.
You're done!
You now have all the required paper work finished, an appointment scheduled and access to the portal. You can then communicate with Dr. Keel via the portal messages, change your appointment date if needed, etc.
You can always come back to the portal now you are a registered patient via the Patient portal button at the top of the website.
Remember your email and password so save it.
