New Patients

Interested in becoming a new patient?

If you are looking to schedule an appointment for an evaluation with Even Keel Health, this is your place.

Step 1:

Complete the appropriate New Patient Registration below

Before you begin....
make sure you have about 10 minutes,
access to your insurance card,
and any prior documentation files such as school reports, IEP/504, or old testing and evaluations
larger screens work best
Make sure you hit submit

Child (5-17 years old)

Adult (18+)

Step 2:

Once the "New Patient Registration" paperwork above has been completed and submitted,

the patient will be registered in our system (EHR). Yes!

As soon as you hit Submit, you will receive an email with instructions on how to register for the portal and complete the final steps

Insurance:

I am contracted with most Anthem Blue Cross Blue Shield, Aetna, and Cigna plans, but not all. It's important to understand that even though I am contracted with these major insurance companies, there are some specific plans within those companies that I am not in-network with. For example, I am in network with most Cigna plans but I am NOT in-network with Cigna's Connect Plans, Local Plus, HMO Connect, and Cigna Surefit plans. While I am contracted with most Anthem and Aetna plans, there are a few unique versions of Anthem and Aetna plans that I am NOT contracted.

Therefore, it is your responsibility to verify with your insurance plan that I am in-network. If your plan considers me out-of-network, you will be responsible for the full cost of the visit. I will provide you with a superbill that you can submit to your insurance provider for possible out-of-network reimbursement.

PLEASE VERIFY WITH YOUR INSURANCE PLAN THAT I AM INDEED IN NETWORK.

For those who are out of network, I do supply a Super bill that you can file with your insurance for possible reimbursement. Those plans would operate as a cash based practice.
